In order to give you the best care possible, we need to collect some information, have you sign some forms and recommend pre-visit instructions. If you are unable to complete the New Patient Registration from your home computer, we have a computer in the waiting room you can use. Please arrive 15 minutes earlier to your appointment.

The first step is to fill out our patient registration paperwork online. This is a secure connection straight to our medical records. Collecting the information ahead of time helps patients move more efficiently through our office.

Follow the link to sign required documents including our HIPAA, Office Policies and Procedures. The forms will be sent to us electronically and we will add them to your file.
What to Bring With You to Your Appointment
- Any insurance cards issued to you.
- Your driver’s license or state issued ID
- Current glasses
- Any contact lens prescription information
- Any ocular history you may have such as cataract surgery dates, lasik surgery dates, etc.
Yea! You’re in! Now, take a moment to visit our online eyewear gallery where you can try on our inventory from home. Create and submit a wishlist of your favorites. Tariq and Kyle, our optical team, will pull the frames you choose as well as some other possibilities for your scheduled appointment. Patient’s who submit a wishlist will receive 15% off their order. This discount is only applicable to cash pay patients, we cannot combine discounts with any vision insurance plan.

At this time, we are requesting that only the appointed patient attends their exam at SeePort. Parents of minors and caregivers are always welcome, of course.

If you are ill or have a fever, kindly reschedule any routine eye care at this time. If you have infection, pain, swelling, or other medical concerns, we can certainly see you via telemedicine if possible.
